As the LPN Clinical Liaison (Health Navigator), supporting the Redstone mission will include the following responsibilities:

• The position reports directly to the Director of Health Navigation. They serve as the main point of contact for clients within the 3 Redstone Highlands Communities in Murrysville, North Huntingdon and Greensburg to understand their needs, providing support, and ensuring client satisfaction. The position may be responsible for conducts clinical rounds with physicians and visits within the local hospital systems. They promote the image and philosophy of Redstone@Home.
• Customer Service:
  • Conduct regular meetings with referral sources.
  • Identify needs of community seniors and referral sources.
  • Coordinate education and program services.
  • Participate in networking activities and events.
• Quality and Compliance Management:
  • Collaborate with internal teams to address client concerns.
  • Support quality and regulatory compliance with partner facilities.
  • Maintain knowledge of funding and government regulations.
  • Utilize technology to plan and document marketing activities.
  • Participate in department and program planning meetings.
• Other Responsibilities:
  • Provide clinical staffing relief for Home Health.
  • Participate in marketing/sales activities.
  • Participate in on-call programs.
  • Perform other duties as assigned.

What do I need to qualify for this role with Redstone?

• A valid nursing license in the state of Pennsylvania.
• Previous experience in home health care/hospice/community health.
• Experience in sales and marketing preferred.
